End Of Life Care

Here at Falcon-Care we understand the difficulties and emotional experiences individuals and their families and friends go through when they are told they or a loved one’s illness is terminal. End of life care also known as palliative care allows your loved one to pass away peacefully in their own home with dignity in the surroundings, they are familiar with and as comfortable as possible.

Falcon-Care provides the support needed in the end stages of someone’s life allowing them to pass in the most peaceful and comfortable way surrounded by their loved ones. Our end of life care can be put in place quickly by our management team and is tailored to each individual taking into consideration their symptoms, wellbeing, social requirements and spiritual needs and wishes.

Here at Falcon-Care our carers can offer the support with many tasks to make your loved one comfortable and safe during this very distressing time.

  • Personal care – supporting with bathing and showering or head to toe wash, dressing needs
  • Continence care – support with toileting and continence needs also looking at skin integrity ensuring continence lesions are prevented
  • Mobility support – such as help moving position in bed and help with standing or sitting – and using hoists when required and other moving and handling equipment.
  • Medication support – administering or prompting medication, and supporting with pain management
  • Fluid and nutrition – support preparing meals and snacks to taste and dietary requirements and support to prevent malnutrition and dehydration
  • Emotional support – Ongoing companionship and emotional support
  • Suitable care according to needs including complex needs – Improve and enhance customers quality of life
  • Religious and cultural support – supporting the emotional, social, and spiritual aspects of the customers’ care.

Here at Falcon-Care your dedicated key worker will work with you and your family to understand and create a care plan to meet your specific needs and wishes, We will discuss present wishes and preferences and also after care of what support you would want from Falcon-Care after the passing of yourself or a loved one, This could be to support with presentation or contacting funeral directors and to give emotional support to family.

There are many options for end-of-life support at home. Most importantly, a care package is created that is completely unique to you or your loved one’s needs, with an expertly trained carer who you feel most comfortable with.

Home Care Services
Falcon-Care were there to offer us support when my Mum passed away. The after care support was above and beyond they helped us and were there to talk if we needed them.

The options for end-of-life care at home include

  • Live-in or 24hour care for end of life
  • A live-in carer will stay in the home with you, being on-hand to provide the support that is needed when it’s needed most. We can also provide 24 hour care where there will be a swap over time of staff at 8am and 8pm.

    Your care manager will work with you to carefully choose one of our fully trained carers, finding the person who is the right fit for your family. This carer will offer continuous support and be someone that you and your family get to know: a familiar face and source of encouragement and comfort.

    Live-in care can be arranged for as little as 24hours. You will just need a spare room for your carer to sleep in. However if you choose 24hour care with a swap over time, carers will not require a room as the night staff will be a wake and watch.

  • Home visits at set times
  • Falcon-Care provides visits for set times each day or week. These can be from as little as 30 minutes, all the way through to several hours at a time or a few visits each day.

    Among many other things, visiting care offers a particularly useful sitting service if a family member needs a few hours of respite relief to ensure they have some down time themselves. Other families use it for extra support with housekeeping or cooking meals or shopping support so they can concentrate more time and energy on those precious final moments together.

    Some people feel more comfortable with receiving personal care from a trained carer rather than a family member or friend. All our carers go through thorough training to offer personal care support in a way that respects personal space and preserves their dignity at all times and also have End of Life and Palliative care training.

  • Overnight end of life care at home
  • If your loved one needs constant support overnight, maybe with medication and pain management or going to the toilet, you can also arrange overnight care. It is a valuable service allowing you, or another caregiver, to have a much-needed rest yourself and also the reassurance that a trained carer is on hand throughout the night.

  • This support can come in the form of either:
  • Waking night support: for constant support throughout the night to ensure each moment
    Sleeping night support: for the peace of mind of someone being in the house if your loved one does wake up and require support
    Emergency care at home: Rapid support to be in place within a short period of time including any of the above support from 30minute pop in visits to over night stays
