- Assess, plan, implement, and evaluate care for patients with complex and multiple health needs.
- Deliver specialist nursing interventions, including wound care, medication administration (e.g., intravenous therapy), and palliative/end-of-life care.
- Perform comprehensive health assessments, including physical, psychological, and social factors.
- Monitor patients’ progress and adapt care plans in response to changing conditions.
- Multidisciplinary Collaboration
- Liaise and coordinate with other healthcare professionals, including GPs, therapists, social workers, and hospital teams, to ensure integrated care.
- Act as a key worker for patients with long-term or complex conditions, facilitating regular team meetings to review care plans.
- Patient Advocacy and Education
- Support patients and families in understanding diagnoses, treatments, and care options.
- Promote self-care and independence through education and health promotion.
- Advocate for patients’ preferences and rights within the healthcare system.
- Leadership and Supervision
- Provide guidance and mentorship to managers and staff.
- Contribute to the development of care protocols and best practices within the team.
- Participate in training sessions to enhance team skills in managing complex cases.
- Quality Assurance and Compliance
- Ensure adherence to relevant clinical guidelines, policies, and legal requirements.
- Maintain accurate and up-to-date patient records.
- Contribute to audits and quality improvement initiatives to enhance service delivery.

- Qualifications and Experience
- Essential:
- Registered Nurse (Adult, Learning Disability, or Mental Health) with valid NMC registration.
- Significant post-registration experience in community or complex care.
- Experience in managing patients with long-term conditions, palliative care, or advanced wound care.
- Desirable:
· Postgraduate qualification in a relevant specialty (e.g., Community Nursing, Palliative Care).
· Experience in leadership or mentorship roles.
· Training in advanced clinical skills, such as venepuncture, catheterization, or PEG feeding.

- A valid UK driver’s license and access to a vehicle will be required.
- Flexibility to work evenings, weekends, and on-call shifts, as necessary.
